Please bring your medical card (if you have it) to the surgery. We shall also require some proof of photographic identity eg passport or driver's licence. There are several forms to be completed, eg for information on previous medical history, data protection, ethnicity and children’s immunisations. Whilst you may register with the doctor of your choice, and you may choose which doctor to consult at any time, you may find that your chosen doctor is not available on a particular day or time of your choice due to the fact that all doctors work at the different sites at different times. All newly registered patients are asked to make an appointment with the practice nurse or healthcare assistant for a routine health check. This allows us to form some background knowledge of you in the interim period before your medical records arrive, which may be several weeks in some cases, and can be particularly important if you suffer from a chronic illness such as diabetes or coronary heart disease. It also gives you the chance to ask any questions about the practice and/or your health care.
